A.  In response to the clarifications and suggestions made during the Pre-bidding Conference, the following revisions in the          Bidding Documents were made for the above-captioned project:

     1.  Delete page 1 of the Invitation to Bid and substitute the herein Attachment Sheet No. “1”;

     2.  Delete pages 22 and 23 under Bid Data Sheet and substitute the herein Attachment Shee Nos. “2” and “3”;

     3.  Delete Section VI. Schedule of Requirements of the Bidding Documents and substitute the herein Attachment Sheet Nos. “4” and “5”.

     4.  Delete pages 1 to 4 of Section VII. Technical Specifications, and substitute the herein Attachment Sheet Nos. “6” to “9”; and

     5.  Delete pages 36 and 37 under Checklist of Technical and Financial Documents and substitute the herein Attachment Sheet Nos. “10” and “11”.

B.  With regards to other clarifications made during the Pre-bidding Conference, please take note of the following:

     1.  The Approved Budget for the Contract (ABC) of the above-captioned project in the amount of Thirty Five Million Two Hundred Ninety One Thousand and Forty Pesos (PhP35,291,040.00) shall remain the same;

     2.  NIA will provide the BIR Form No. 2307 – Certificate of Creditable Tax Withheld at Source;

     3.  The supporting document for the Statement of Single Largest Completed Contract (SLCC) Similar to the Contract to be Bid shall be limited to the following:

              a.)  End User’s Acceptance, or

              b.)  Office Receipt, or

              c.)  Sales Invoice

C.  Receipt of this Notice must be acknowledged by the Bidders at the Office of the BAC-A Secretariat, 6th Floor, NIA Building “A”, EDSA, Diliman, Quezon City.

D.  This Notice shall form part of the Bidding Documents.
